- Patient name
- Date of birth (must be at least 18 years of age at time of referral)
- BC Provincial Health Number (PHN)
- Patient telephone number (preferably mobile telephone number - for receiving text messages)
- Patient email address (referral automatically rejected if not included)
- Referring clinicians name, MSP number, and fax number
- Only referrals faxed directly from the clinician's office are considered (not from the patient)

Clinicians: If your faxed referral arrived at my office you will have already been notified by fax (accepted/rejected – with explanation) within two business days of the date you originally faxed the referral. Do not automatically assume that faxing a referral means that the referral actually arrived at my office. All you need to do is check your own records to see if you received a faxed notification from my office. If you did not receive a faxed notification from my office, it means that your referral never arrived (and therefore your patient is not in my system, does not have an appointment, and is not on the waiting list at my office). - Wait at least two weeks after the referral was sent to receive an email from my office. Check your email spam folder. If the email is not in your spam folder and it has been over two weeks, that means your name is NOT in my system.
- Contact the office that attempted to send your referral to find out what happened. If your referral was successfully transmitted, they will have already received a faxed response back from my office. In particular, ask: "DID YOU RECEIVE A FAXED RESPONSE FROM THE OFFICE OF Dr. EMES?" If the answer is...
- No - Your faxed referral never arrived at my office. This is the most common reason, as my office fax servicce has only limited availability. See TROUBLE TRANSMITTING FAXES for more information.
- Yes – Did the faxed response state that the referral was rejected? The most common reasons for rejecting referrals are; missing patient email address, missing other essential information, illegible document, referral sent by email, or referral sent through the postal service. When appropriate, my office will advise the referral source on how to resubmit the referral. In order to ensure patient confidentiality, my office does not keep the record of rejected referrals. The only way to learn if the referral was rejected is to check with the referral source.
- Yes – Did the faxed response state that the referral was accepted? If so, please have the referral source contact me immediately (email/telephone/fax) to confirm the status of your referral. Your referral source should be able to provide you with a copy of the faxed acceptance sent from my office.

The following are services provided by Dr. Craig Emes;
The office does NOT provide the following services;
- Comprehensive Adult ADHD Assessment with Treatment Recommendations via telehealth
- Detailed consultation report with treatment recommendations sent immediately to the primary healthcare provider
- Collaboration with the primary healthcare provider to ensure effective treatment
- Telephone advice to the referring primary healthcare provider (or clinic) for up to six months after initial consultation, as needed
- Primary healthcare provider (or clinic) provides ongoing patient care, including prescribing psychostimulant medications as indicated
- Reassessment every six months, if required
- Postsecondary educational accommodation and financial assistance forms (ADHD Verification and Appendix 8)
- ADHD Medication financial assistance strategies (to eliminate the consumer cost of recommended ADHD medication treatment)
The office does NOT provide the following services;
- Private pay consultations
- In-person appointments (telehealth only)
- Consultations for those without BC provincial insurance coverage (must have BC PHN)
- Consultations for those without a referral from healthcare provider registered in British Columbia (must MSP billing number)
- Assessment/treatment of Autism or other mental health conditions
- Prescription medication initiation or continuation
- Ongoing individual treatment or follow up care
- Completion of work, insurance, or tax-related documents
