specific referral status
All accepted referrals are placed on a waiting list until time slots are available. At that time, patients are emailed an Online Assessment Form. This process usually takes less than two weeks. After the office receives the completed Online Assessment Form, the first appointment is usually within 2 to 4 weeks.
If patients have not already received an email from my office (consider checking SPAM folder), it means either (1) they are on the waiting list, (2) the referral was rejected, or (3) the referral never arrived at my office.
For inquires about if a faxed referral was received at my office; please check with the referral source - if the referral arrived at my office the referral source has already been notified by fax (accepted/rejected – with explanation). If they did not receive a faxed notification from my office, it means that the referral never arrived.
